Q.  How do I become a registered medical marijuana user in Colorado?

A.  In accordance with Colorado's Amendment 20, a patient who has been diagnosed with a debilitating condition that may be alleviated by the medical use of marijuana may apply to the Colorado Department of Public Health and Environment for a Medical Marijuana Registry identification card. The patient must obtain certification from a physician licensed in Colorado that he/she has been diagnosed with a debilitating condition that may be alleviated by the medical use of marijuana. The patients should submit an application along with the Physician Certification form, a copy of a photo ID, and the non-refundable $90.00 application fee to the:

Colorado Department of Public Health and Environment
HSVR-ADM2-A1
4300 Cherry Creek Drive South
Denver, Colorado 80246-1530

The Registry will verify the licensure status of the physician and may contact the physician to verify the certification. Verifications will occur within 30 days of receipt of the application.
The Registry will mail the patient a Medical Marijuana Registry identification card within 5 days after the verification.
The patient will also be notified within 5 days after the verification process if the application has been denied.

Q.  Which conditions are considered debilitating?

A.  Cancer; glaucoma; HIV/AIDS positive; cachexia; severe pain; severe nausea; seizures, including those that are characteristic of epilepsy; or persistent muscle spasms, including those that are characteristic of multiple sclerosis. In addition, patients and physicians may submit petitions to the Colorado Department of Public Health and Environment to include other medical conditions that may be alleviated by the medical use of marijuana.

Q.  What should I do if my regular doctor is unable to write a recommendation?*

A.  You should have your medical record reviewed by a compassionate physician, who may be willing to write a recommendation, if he/she feels that your condition may be relieved by use of medical cannabis.*

Q.  Where do I find a compassionate physician?*

A.  There are several alternatives.  The three widely advertised clinics are Rocky Mountain Patient Services at 720-882-5521, CannaMed 877-420-MEDS and THCFoundation 303-403-9996.*

Q.  What is the definition of a Primary Care-giver?*

A.  According to Amendment 20, "...Primary care-giver means a person, other than the patient and the patient's physician, who is eighteen years of age or older and has significant responsibility for managing the well-being of a patient who has a debilitating medical condition...”. In other words it may be interpreted as a person who may be delegated by you to grow, obtain, or in any other way provide your medicine.  As long as "...The patient and his or her primary care-giver were collectively in possession of amounts of marijuana only as permitted under this section..." which is "No more than two ounces of a usable form of marijuana; and No more than six marijuana plants, with three or fewer being mature, flowering plants that are producing a usable form of marijuana..."*

Q.  What are the benefits of having a caregiver?*

A.  If you are unable to grow your own medicine in quantities and varieties needed, it may be beneficial to sign over the care-giver status to a person who is a member of a collective or a dispensary staff member.  This will assure safe and affordable access to a wider variety and amounts of medication necessary for your condition.  Since the dispensary/collective may then legally obtain, store, and maintain medicine on your behalf with you having access on "as-needed" basis.*
